Making a new patient appointment
- To schedule a new patient appointment, you can call or stop in to provide us with the information we need to schedule you!
- When calling or stopping in, please have the following information ready: your personal information (name, date of birth, phone number, address), diagnosis/body part we are treating, referral information, and insurance information (insurance carrier, member ID number, insurance phone number)
- We require patients to have a referral from a doctor within the last 30 days
- If your insurance plan has Direct Access, and you have not been seen by a doctor or other health care professional for the same condition in the last 60 days, then you do not need a referral
