Making a new patient appointment


  • To schedule a new patient appointment, you can call or stop in to provide us with the information we need to schedule you! 


  • When calling or stopping in, please have the following information ready: your personal information (name, date of birth, phone number, address), diagnosis/body part we are treating, referral information, and insurance information (insurance carrier, member ID number, insurance phone number)


  • We require patients to have a referral from a doctor within the last 30 days


  • If your insurance plan has Direct Access, and you have not been seen by a doctor or other health care professional for the same condition in the last 60 days, then you do not need a referral

initial evaluation


  • Arrive about 15 minutes early to allow for ample time to check in and fill out paperwork!


  • Bring a photo ID, insurance card(s), prescription/referral, and medication list (if applicable)


  • For an easier check-in upon arrival, fill out the new patient paperwork and bring it with you to your first appointment! There may be additional forms depending on your insurance.




follow-up appointments 


  • At the end of your initial evaluation, our therapists will recommend the frequency and duration for your follow up appointment


  • Follow up appointments can be made over the phone or in the office

Cancellation policy


  • We enforce a cancellation policy and require 24-hour notice for cancellations. There is a cancellation fee of $50.00


  • If you feel you have received a bill in error, please give us a call


  • Co-pays are collected at the time of service and can be paid with cash, card, or check


  • We do not accept American Express or Discover
